After hours support
We ask that routine questions, prescription refills and appointments be made during regular business hours. For a life threatening emergency, call 911 immediately. For other urgent medical needs when the office is closed, you may call our office at 704-384-1260 and your call be forwarded to the after-hours triage support team.
Our office welcomes new patients from newborns through seniors and everyone in between! You may call 704-384-1260 between the hours of 8 a.m. and 5 p.m. Monday through Friday to schedule your new patient appointment. We participate with most insurance plans. However, we recommend that you check with your insurance provider to make sure that you are able to come to this office. Most insurance companies provide an 800 number on the membership card to a customer service representative that can assist you in verifying this information.
If you are transferring from another medical practice, please contact that office and request that your medical records be sent to Novant Health Crown Point Family Physicians. We strongly encourage patients to schedule a visit before illness occurs. This gives you an opportunity to learn about us and for us to get familiar with your medical history.
Please bring your insurance card with you for each appointment. All insurance copayments and deductibles are due at time of service. Copays will be collected before you are seen. It would be helpful to arrive a few minutes before your scheduled appointment time to allow for the check-in process.
Confirmation & cancellation
Our Interactive Voice Response system, automatically calls most patients two days prior to their appointment. You may confirm or cancel an appointment on the phone at this time. We do require 24 hour notice to cancel your appointment.
Copays & deductibles
All insurance copayments and deductibles are due at the time of service.
Our referral coordinators will assist you with making appointments for outside tests and specialists. If your insurance plan requires a referral or pre-authorization, they will obtain it. If it's not required, you may be given information to make your own appointment at a time that is most suitable for you. Please allow 72 hours when authorization must be obtained. Retroactive referrals are not permitted. We encourage you to be proactive and call your insurance company for coverage guidelines, especially for diagnostic testing (i.e. CT scans, MRI, etc).
