Geriatric care

Geriatric care deals with diseases and disorders affecting adults as they age. At Novant Health Crown Point Family Physicians, we work to provide consistent medical care to our geriatric patients by monitoring ongoing health and addressing any health issues as they happen.

You can rely on Crown Point Family Physicians to make necessary referrals and help coordinate your care.

Medicare & annual wellness visits

Medicare patients are eligible for a free “welcome to Medicare” visit within the first 12 months after your enrollment into Medicare.  After your first year, Medicare offers a free “annual wellness visit.”  These visits are designed to help you stay well.

You can expect your medical and family history, current health conditions and prescriptions to be reviewed.  We will also check your blood pressure, vision, weight and height and ensure that you are up to date on preventive screenings and services, such as cancer screenings and shots.

Your nurse and doctor will require extra time in the room to complete several forms.  Please indicate your desire to schedule a “welcome to Medicare exam” or “annual wellness visit” when you call.  We will be certain to block enough time with the doctor and prepare the necessary forms.

Note: This is not a complete physical with blood work.  Medicare does not cover a regular physical.  If you have chronic conditions (diabetes, cholesterol, hypothyroidism, etc) that require blood work, your doctor may order blood work that can be supported by a diagnostic code.
